You need to complete a household services claim form (D9319) and take to your treating doctor to complete. The completed form will need to be emailed to hhs@dva.gov.au. If you are eligible for an assessment, an occupational therapist will be requested to conduct a home-based assessment with you. Following the assessment, you will be provided with a determination of eligibility of services.
